Wheelchair license Plate: Drop Box Instructions

Please allow 5-7 business days for processing.

Drop boxes are located in all of our service centers. Please submit the following items in a sealed envelope and place it in one of our drop box locations:

1. Completed form HSMV 83007, Application for International Wheelchair License Plate, or form HSMV 83039, Application for Disabled Parking Permit.
2. A copy of the vehicle registration certificate.
3. Copy of Florida driver license or ID card.
4. Proof of Florida insurance for the vehicle.
